The NurseNavigator position is Monday-Friday, 8-5 with some days being longer to assistwith covering clinic. This role isspecifically for our Renal clinic and is a high functioning position;background with renal patients is a plus. The nurse will be in charge of running clinic for their providers,educating patients and helping patients with their questions/concerns (viain-basket through MHC and/or phone calls). Teamwork is essential as the Renal clinic is housed in theMulti-Specialty clinic; peers include PARs, MAs, RTs and other RNs.
Serves as apatient advocate, liaison, and advisor/educator to assist patients withnavigating the continuum of care.
Work Schedule:Full Time Days
Roundson/reviews assigned patients regularly and evaluates patient progress with planof care. Communicates plan of care to patient and family and solicits concerns,questions, and issues for resolution.
Providesemotional support, counseling, clinical education and expert guidance topatients and families to promote their ability to understand and meaningfullyparticipate in the healthcare process and personal decision-making.
Serves as aliaison between patient and clinical staff, administration, physicians, managedcare companies, and community/external resources to coordinate/maximizeresources and identify/resolve barriers to the plan of care.
Assistspatients and families with resolving financial, psycho-social, functional, andadministrative issues by advising of options and referring to appropriateresources. Intervenes as appropriate to advocate for patient and family.
Within scopeof job, requires critical thinking skills, decisive judgement and the abilityto work with minimal supervision. Mustbe able to work in a fast-paced environment and take appropriate action.
Minimum Required Education:Graduate ofan accredited Registered/Professional Nursing program if less than 3 yearsexperience.Preferred:Bachelor'sdegree in Nursing.
Required Licensure/Certification:Statelicensure as a Registered Nurse (RN). Relevant life support certification asdetermined at position level.
Minimum Experience:2 years of nursingexperience.
BLS throughthe American Heart Association or the American Red Cross CPR for theProfessional Rescuer with card in-hand before start date. BLS or CPR card must be good through sixtydays of hire. All life supportcertification cards must be issued by either the American Heart Association orthe American Red Cross CPR for the Professional Rescuer. (American Red Crossfor the Professional Rescuer is only good upon hire and all renewals will needto be through an American Heart Association sanctioned course.)
